Schedule Well Checks/Sports Physicals – No On-Campus Option This Year

Dear Families,

This year, we will not be offering on-campus sports physicals. Instead, SLV Health has reserved appointment times the week of July 28 at both the Monte Vista Clinic and the Stuart Ave Clinic in Alamosa for well child checks and sports physicals.

Appointments will be billed to insurance as a well child visit unless:

• Your insurance does not cover wellness visits, or
• You are uninsured

In those cases, a sports physical will be provided for $37. Be sure to mention what type of appointment you need when scheduling.
